Our email address is       matchinggreensurgery@nhs.net  This email can be used ONLY for administrative requests eg. repeat medication requests, sick note requests or any information that has been requested by your GP. If you have a clinical enquiry or an urgent clinical problem, you must call the surgery and not leave an email as this will not be monitored.

Access to the Surgery

Access to the building will be restricted to the patients who have been invited for an appointment by a Clinician.

All routine appointments with a GP will be telephone consultations.  If the GP requires patients to be seen face to face , you will then be offered an appointment to come to the Surgery for a doctor to assess you. 

Some nurse appointments may be telephone consultations . You will be advised if this is the case. 

Anyone who attempts to gain access to the surgery who has not been invited will be asked to leave the premises.

Only the patient with the appointment will be allowed into the building.

For children, only one parent/guardian will be allowed to accompany the child.

Please wash your hands in the patient toilets when entering the building. 

COVID is still around, and particularly with the risk of the new India variant rising, having lots of unwell people close together in the waiting room isn't safe either. 

We will keep you updated as soon as we are clear about how we can re-open our reception areas safely. Thank you for your support.

Data Sharing and Data Opt-out 

Type 1 opt-out & National Data opt-out

Your health records contain a type of data called confidential patient information. This data can be used to help with research and planning.

 

You can choose to stop your confidential patient information being used for research and planning. You can also make a choice for someone else like your children under the age of 13.

 

NHS Digital will never sell your data. There are strict rules about how NHS can use your data. It's only shared securely and safely. Shared data helps the NHS. It has been used to find the first treatment for coronavirus and for vaccine research.

 

If you're happy with your confidential patient information being used for research and planning you do not need to do anything.

Opting out

If you don’t want your identifiable patient data to be shared for purposes except for your own care, you can opt-out by registering a Type One opt out or a National Data Opt out, or both. These opt-outs are different and they are explained in more detail below. Your individual care will not be affected if you opt-out using either option.

Type 1 Opt-out (opting out of NHS Digital collecting your data)

NHS Digital will not collect data from GP practices about patients who have registered a Type 1 Opt-out with their practice.  You can opt out by  completing the form and send to your GP practice.

Type 1 Opt-out form

This collection will start on 1 September 2021. 

If you register a Type 1 Opt-out after this collection has started, no more of your data will be shared with NHS Digital but will however still hold the patient data which was shared with them before you registered the Type 1 Opt-out.

If you do not want NHS Digital to share your identifiable patient data with anyone else for purposes beyond your own care, then you can also register a National Data Opt-out.

National Data Opt-out (opting out of NHS Digital sharing your data)

NHS Digital will collect data from GP medical records about patients who have registered a National Data Opt-out. The National Data Opt-out applies to identifiable patient data about your health, which is called confidential patient information.

NHS Digital won’t share any confidential patient information about you - this includes GP data, or other data we hold, such as hospital data - with other organisations, unless there is an exemption to this.

GP Online Service

Save time – do it online! GP online services gives patients the option to book appointments, request repeat prescriptions and view their GP records, using their computer, tablet or smartphone rather than having to phone or visit their practice.

Online services complement the existing ways patients can access appointments, prescriptions and their records. Our practice already has the facility to offer online access to records and transactional services for our patients.  

If you are not already registered for GP online services please let a member of the practice team know you would like to use this service.

 

Named accountable GP for all patients

As of 1st April 2015 all our registered patients have an allocated named GP to oversee their care

Dr R Chandel is your named GP who has the overall responsibility for the care that the surgery provides to you.

You can still see any GP working at the Practice and please continue to book appointments as you do with any of our available GPs or nurse


 Partners in Care

Once registered, patients and healthcare professionals work together to ensure the most appropriate care is provided. This partnership philosophy extends even further and our active patient group exists to make sure that patient needs and the practice offering are always heading in the same direction.

The surgery has suitable wheelchair access for disabled patients.
