New Patient Registration

All new patients wishing to register should collect a GMS 1 form from reception. You will also be required to complete our practice health questionnaire at the time of application.

For children or young person under the age of 18 please fill the "REGISTRATION FORM FOR CHILD OR YOUNG PERSON"

All the forms can also be downloaded from the above "Registering Online" tab. 

 We require two types of identification with your application form – a photo ID (e.g. driving licence or passport) and the other should confirm your address (e.g. utility bill). 

Please bring the registration form (if applicable the Child and Young person form), New Patient Questionnaire and the identification documents to the Surgery between 12:00 - 14:00

When accepted please make an appointment to see our practice nurse for a New Patient Medical – your registration will not be complete until you have attended this medical. 

Please bring a specimen of urine for testing (a bottle will be provided on request) and also any medication you have been taking.

Alternatively you may print off 

• registration form,
• registration questionnaire
• and if there are children under 18 yrs old - New child or Young person form 
• fill the relevant forms and bring it in with you on your first visit to the practice with the above mentioned two types of identification.

Change of Address or Telephone Number

If you change your telephone number or address, please let us know as soon as possible in case we need to contact you in an emergency.
